2025-2026 TRYOUT PACKET
TEACHER EVALUATIONS
Teacher evaluations will be averaged together to count for a portion of each candidate’s score. Only those teachers who taught the candidate during the 2024-2025 school year will participate in the evaluation process. Therefore, it is very important to give the student an honest rating based on your knowledge of that individual student.
Please complete this form by April 17th, 2025. When submitting this form, it will go directly to Coach Mia’s email (miakdavi@gmail.com)
It is very important that you rate the student according to how YOU feel the student does in YOUR class(es). Please be realistic as well as fair. These evaluations will not be shared with the student. They will be confidential and tallied by the Coach.
